  • I have been informed that this is not a medical assessment or diagnosis, nor a substitute for professional medical advice or treatment
  • I have either received clearance from a physician to participate in physical activity or choose to proceed at my own discretion
  • I am solely responsible for monitoring my condition and stopping participation if I experience pain, dizziness, or discomfort
  • I understand and accept the inherent risks involved in voluntary participation
